Exhibitors' Registration Options - MnDHA Annual Session '09

Please note:  To use reserve space at the Annual Session using a credit card please follow the instructions. 

1.  Fill out the registration below.  In the Comment box, please include your Company Name.  If you will be selecting the  Breakfast and Learn, add the Topic and Speaker.  For Silent Auction items, please list with a comma in between items.  Once you are finished, click the "Submit" button on the bottom of the form

2.  Select your exhibit options from the "Items" link on the left hand side of the page.

3.  If you have a Paypal account, you will be using the PayPal Express option.  At this time, we have no other credit card option.
